Shining Stars of America Target Case Managers will use a personal approach to social welfare, perform comprehensive intake assessments, develop Individualized Work Plans, provide information and referral to other resource agencies, serve as advocates for services, monitor trainee and progress in achieving case services objectives, and provide direct counseling.

The overall goal of Shining Stars of America Target Case Management Services is to provide attentive and compassionate human services such that our trainees and colleagues can achieve, sustain, and develop the independence that comes with having a job.

The purpose of Shining Stars of America Target Case Managers is to provide the following services:

Orient to the concept of consumer-direction

Provide training skills on how to recruit, interview, hire, evaluate, manage or dismiss attendants in the home

Provide support services as directed by DAAS (Division of Aging and Adult

Services)

Assess clients’ needs to assist in accessing services that currently may or may not be in place in the home.

Monitor Alternatives participants’ needs, employer status and circumstances, and report findings according to program policy to DAAS.

Refer the client to resources, such as energy assistance, legal assistance, emergency housing and others, to assist in meeting their needs.

Schedule appointments for the client to help them gain access to medical, social, educational and other services appropriate for the client’s needs. This includes, but is not limited to, medical appointments, transportation services and appointments with DHS.

Perform face-to-face or telephone contacts with the client and/or other

individuals to assist in meeting the client’s needs.

Assist the client in completing the application for assistance.

Train assistance care providers in proper billing procedures.

Explain the current program policy and monitoring compliance
